CLUB RULES

1. Completion and submission of this membership application is required prior to participation.

One complimentary practice session will be provided.

2. All players must wear soccer cleats and shin guards at all practices. Players without

proper equipment will not be permitted to participate.

3. A PFC Warriors practice jersey is required at every practice. One jersey will be issued at

registration for an additional fee; replacement jerseys may be purchased.

4. Administrative club fees are due between the 1st and 5th of each month. A late fee of

$10.00 will be applied after the 5th. Players will be suspended from practice beginning on

the 10th of the month until all outstanding fees are paid.

5. All administrative fees are due regardless of player attendance. A medical disability lasting

more than 30 days accompanied by a doctor’s note may qualify for a credit toward the next

month.

6. Respectful behavior among players, parents, guardians, administrators, and coaches is

mandatory. Inappropriate conduct will result in disciplinary action as deemed necessary.

7. A parent or legal guardian must remain present at all practices and games. PFC Warriors

assumes no responsibility for the supervision of the child outside the organized activity

itself.

8. Parents must report any illness, medical condition, or medication taken by the participant

directly to the Division Coach.
